Returning to the Decision Support Tool

1. On the CPM-PTS Background Information and Case Link page you will see the following note:

2. In the Decision Support Tool, you will find a message at the bottom of each page. Be sure to capture the unique URL by taking a screenshot, copying it, or printing the page with the URL. This will allow you to easily return to the Decision Support Tool. Sample: with URL

3. To return to the DST, simply use the copied URL. Upon opening the link, you will be prompted to log in by entering the Case Identifier, the Date of CPM administration, and the Name of the staff member listed on the CPM-PTS Background Information and Case Link page. Once entered correctly, note that responses are not case-sensitive, you will have access to the related DST responses.
